• print out the form you need;
• fill out completely your part;
• sign it;
• attach required documents, if any;
• collect required signatures (of your instructor, your advisor, etc., if any) in the specified order;
• give it to the Department Chair, Dr. Plaza, leave it in his mailbox or in Redcay 103, or fax it to (518) 564-3010.
